The Name field is a set of text fields where people can enter their name.

Field Limits

The First and Last fields each accept any text up to 256 characters.

Field Settings

The Name field type supports the following Field Settings:

• Require an Answer
• Admin Only Field
• Instructions for User
• CSS Layout Keywords

Name Format

You can change the Name Format option to Extended to add a Title and Suffix field.

If we don't offer the format you need (for example, if you want to add a middle name field), or you want to be able to change the field labels, you can create your own custom name field by placing two or more Single Line Text fields side by side using CSS Layout Keywords.
